Physical Therapist (PT) Job Requirements

• Minimum 1 year of experience as a licensed Physical Therapist (PT)
• At least 6 months of experience in a Home Health setting as a PT (home health PT jobs preferred)
• Proficiency with OASIS documentation and HCHB (Homecare Homebase)
• Current Physical Therapist license in the state of Arkansas (AR)
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit

Home Health Physical Therapist Responsibilities

• Conduct comprehensive home health physical therapy evaluations and skilled treatments in patients' homes
• Develop and implement individualized care plans focused on functional goals, fall prevention, and safe discharge outcomes
• Accurately document visits, progress notes, and OASIS assessments using HCHB
• Collaborate with case managers, nurses, and interdisciplinary team members to optimize patient outcomes
• Educate patients and caregivers on therapy techniques, home safety, adaptive equipment, and caregiver training
• Maintain timely, compliant documentation of patient progress and outcome measures
