Personal Outcome Measures Interviewer – JOB DESCRIPTION
RESPONSIBLE TO: QA/QI Supervisor
FUNCTION:
To assist the organization in helping people discover and define their own quality of life as well as identify quality of supports provided to people supported by the organization by conducting POM (Personal Outcome Measures) interviews
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Strong observational skills. Must have the ability to multi-task with strong organizational skills. Demonstrates strong interpersonal skills with patience and empathy. Independent thinker with the courage to stand alone and report unbiased information.
MINIMUM QUALIFICATIONS:
High school diploma or equivalent and 5 years’ experience working with people with disabilities and/or mental health diagnoses. Associates degree or DSP/FLS Certification preferred.
Ability to become CQL Certified as a POM interviewer within 1 year on the job and ability to maintain the certification.

DUTIES
- Coordinates all interview activities including scheduling, obtaining consents, conducting interviews and outcome determinations.
- Documents outcome determinations in CQL portal.
- Documents and provides recommendations to person’s team based on interests discovered during the interview process.
- Provides valid/reliable data using instructions provided by CQL.
- Provides data monthly to the CQL Steering Committee.
- Facilitator of “Dignity and Respect” Basic Assurance factor group and coordinates action plan required of accreditation.
