Responsibilities- Provide family-centered case management focused on strengthening family functioning
- Support children and youth in maintaining positive connections with their families
- Deliver culturally and linguistically responsive services to diverse populations
- Conduct home visits and provide individual, family, and group outreach
- Help families access community resources and build natural support systems
- Coach parents on effective communication, supervision, and nonviolent parenting strategies
- Advocate for families in educational, legal, and social service settings
- Assist in stabilizing children in foster care placements
- Transport children and families as needed to appointments and services
- Complete detailed documentation in electronic records for use by social workers, courts, and other professionals
- Work toward achieving service goals within established timelines
- Collaborate with multidisciplinary teams to coordinate care and services
The Ideal Candidate
You are:
- A strong communicator with excellent written and verbal skills
- Detail-oriented and able to manage documentation effectively
- Compassionate and culturally responsive
- Comfortable working independently in the community and in family homes
- Knowledgeable (or eager to learn) about community resources and parenting supports
- Bilingual (a plus, but not required)
- Flexible and able to travel extensively within the service area
- High School Diploma, GED, or HiSET required
- 0–3 years of related experience in human services, family support, or a similar field
- Valid Class C or Class D Driver’s License
- Reliable transportation with current registration, inspection, and insurance
