Title: Case Manager

Location: Kentucky River Community Care.  Letcher County

Credentials: BA/BS plus one (1) year experience

Employment Type: Full Time

Skills: Organization, maintain case load, time management.

Primary Functions:

• To assess for strengths and basic needs of the individual requiring case management services.
• To create a plan with the clients and treatment team for what services the client needs and goals they would like to achieve.
• Help the individual to access needed resources and services.
• Identify and assist in finding support systems for the individual to succeed in recovery.
• Monitor progress the individual is making towards achieving goals.

Shifts/Day and Work Hours: Monday through Friday; 8 am- 4:30 pm.
