Requirements:
- Graduate of school of pharmacy: Required
- Two years' experience as a Pharmacist: Preferred
- Two years' pharmacy leadership experience: Preferred
- Pharmacist license in the state of CA: Required
Description:
- Provides oversight of pharmacy clinical services (CPS).
- Provides guidance for the clinical operational and clinical decision-making aspects of CPS.
- Periodically consults with practitioners in the field.
- Develops, implements, and oversees the CPS patient management program.
- Maintains an updated patient management program policy and procedure.
- Provides guidance to clinical pharmacists regarding all patient management decisions and operations.
- Develops quality improvement programs (QIP) that relate to CPS.
- Identifies and implements quality improvement programs and continuous quality assurance initiatives to advance the medication use systems and promote optimal patient outcomes.
- Develops an outline of the QIP.
- Collects quarterly data to evaluate the progress of the QIP and report results to the various quality committees.
- Oversees the initial and ongoing disease and medication training of clinical pharmacy staff.
- Provides patient care as needed.
