Job Summary:

Job Details:

- Required Qualifications: Master's Degree in Physical Therapy; Wisconsin Physical Therapist License in hand; BLS certification; AHA or ARC certifications; NPI required (must start application if not already obtained)
- Preferred Qualifications: Two years of home health experience highly preferred, with training in adult and geriatric populations, orthopedic, neurological, and functional mobility diagnoses; knowledge of Physical Therapy standards and practices; experience working with electronic medical records (NetSmart EMR); familiarity with regulatory and reimbursement documentation requirements
- Additional Information: Candidates should be hard workers, resilient, able to manage change effectively, and social. The position may include occasional travel to other branches. Typically no weekends or holidays; however, the PTs can rotate one Saturday every 6-8 weeks, with the Saturday worked given a day off during the week. During flu season, unvaccinated candidates will be required to wear masks at all times, with no exceptions.
- Additional Responsibilities: Evaluate, treat, and plan patient care following Physical Therapy Standards of Practice, documenting appropriately to meet regulatory and reimbursement requirements. The expected productivity is an average of 4.5 visits per day.
