About Upward Bound House
UBH ends intergenerational family homelessness through crisis housing, permanent housing, and prevention services. For 30+ years weve led with compassion, equity, and collaboration in a fast-paced, mission-driven, growth-minded environment.



About the role
Join UBHs new CalAIM program supporting families enrolled with managed care plans. Youll deliver community supports (e.g., housing deposits, navigation, tenancy sustainment, day habilitation/skills-building) and partner with landlords to secure and stabilize permanent housing for homeless families.


What youll do

  • Complete comprehensive 5x5 assessments and create Family Service Plans.
  • Provide field-based services, including travel to multiple service locations.
  • Provide trauma-informed case management: referrals, coaching, goal tracking, and wellness checks.
  • Lead the housing process end-to-end: search, applications, landlord communication/negotiation, lease-up, and advocacy.
  • Document services in HMIS, EHR, and internal systems; maintain accurate case files.
  • Conduct habitability/health & safety checks on units.
  • Mediate landlord/tenant issues (late rent, repairs, neighbor concerns) with the housing stabilization team.
  • Collaborate with partner agencies; attend case reviews, community meetings, and required CalAIM trainings.



What you bring

  • Bachelors degree and 2+ years in human services (or equivalent relevant experience).
  • Experience with people experiencing homelessness; families preferred.
  • Working knowledge of fair housing laws and landlord/tenant practices.
  • Excellent communication, problem-solving, and boundary-setting; calm under pressure.
  • Commitment to trauma-informed, culturally responsive care.
  • Team player who can also work independently in a fast-changing environment.
  • Proficient with Microsoft Office and data systems (HMIS/EHR).
  • Valid CA drivers license and auto insurance; ability to work occasional evenings.



Mandatory requirements
Employment eligibility verification, reliable transportation, a current TB test, and a successful background check.



Nice to have (optional)
Bilingual skills; prior CalAIM, managed care, or housing navigation experience.
