The Home Health Aide provides personal care and health-related services to patients in their homes, supporting their independence and quality of life under the supervision of nursing staff.

Qualifications

  1. Completion of a state-approved Home Health Aide training program
  2. High school diploma or equivalent (preferred)
  3. Valid driver's license and reliable transportation
  4. Current CPR and First Aid certification
  5. Ability to read, write, and communicate effectively in English
  6. Physical ability to perform essential job functions

Essential Duties and Responsibilities

  1. Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
  2. Perform light housekeeping duties related to patient care
  3. Prepare and serve meals according to prescribed diets
  4. Assist with medication reminders (as allowed by state regulations)
  5. Monitor and record patient's condition, reporting changes to the supervising nurse
  6. Measure and record vital signs
  7. Assist with range of motion exercises and ambulation
  8. Provide emotional support and companionship to patients
  9. Maintain a safe and clean environment for the patient
  10. Document care provided in accordance with agency policies and procedures
  11. Maintain patient confidentiality and adhere to HIPAA regulations
  12. Participate in care planning meetings as requested
