Job Description
Position Summary:
The LVN Clinical Coordinator plays a key role in supporting the delivery of high-quality home health services by coordinating patient care, assisting clinical staff, and ensuring compliance with regulatory standards. This position bridges communication between field clinicians, patients, physicians, and administrative staff to maintain efficient operations and positive patient outcomes.
Key Responsibilities:
- Coordinate and schedule patient visits for nursing and therapy staff based on care plans and patient needs
- Review and process physician orders, care plans, and clinical documentation for accuracy and completeness
- Act as a liaison between field staff, physicians, patients, and caregivers to ensure continuity of care
- Monitor patient census and staff productivity to support efficient workflow
- Assist with onboarding and training of new clinical staff regarding agency protocols and documentation systems
- Ensure compliance with federal, state, and agency regulations, including Medicare/Medicaid guidelines
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
- Track and follow up on lab results, physician communications, and outstanding documentation
- Support clinicians with problem-solving in patient care situations and escalation when needed
- Maintain confidentiality of patient information in accordance with HIPAA standards
Qualifications:
- Current, active Licensed Vocational Nurse (LVN) license in the state of practice
- Strong knowledge of home health regulations and documentation requirements
- Excellent organizational, communication, and multitasking skills
- Proficiency in electronic medical records (EMR) systems
- Ability to work collaboratively in a fast-paced environment
