Case Manager- Supportive Housing

We are seeking a dedicated and compassionate Case Manager to provide comprehensive social services to vulnerable individuals and families in the Bronx. This role is ideal for professionals who are passionate about advocacy, housing stability, and helping clients achieve long-term self-sufficiency.

Full Time, Monday to Friday

Requirements:

• Bachelor’s Degree (required)
• Minimum of 2 years of case management or social services experience
• Strong documentation and database skills (CARES, Client Track, or similar systems preferred)
• Ability to manage a high-volume caseload (50+ clients)

Key Responsibilities:

• Conduct comprehensive intakes, assessments, and periodic re-assessments
• Develop and implement Independent Living Plans with measurable short- and long-term goals
• Manage an active caseload and provide ongoing client support
• Escort clients to housing, medical, and legal appointments; assist with entitlement applications
• Facilitate referrals for housing, healthcare, and mental health services
• Support housing placement efforts, landlord negotiations, and eviction prevention
• Provide crisis intervention and individual counseling as needed
• Maintain accurate documentation in electronic databases and client files
