Location: Brooklyn & Queens, NY
Schedule: Monday–Friday | Full-Time

We are seeking an experienced Registered Nurse (RN) Coordinator to support healthcare coordination for individuals living in residential community programs throughout Brooklyn and Queens.

The RN Coordinator will oversee participant health management, coordinate services with interdisciplinary teams, and ensure compliance with clinical care plans and regulatory standards.

This role offers a temp-to-permanent opportunity with strong long-term potential. Candidates who transition to permanent status will receive a company vehicle as part of the position.

⚠️ Only candidates with previous experience working within the OPWDD network will be considered.

Key Responsibilities

• Coordinate healthcare services and monitor participant health status

• Conduct nursing assessments and oversee ongoing clinical needs

• Develop and update individualized healthcare plans

• Collaborate with physicians, therapists, case managers, and program staff

• Provide clinical guidance and training to direct support professionals

• Monitor medications, treatments, and physician orders

• Participate in interdisciplinary care meetings and case reviews

• Ensure compliance with OPWDD regulations and documentation requirements

• Maintain accurate clinical documentation and reporting

Must-Have Qualifications

• Active New York Registered Nurse (RN) license

• Previous experience working within the OPWDD system or network (required)

• Experience supporting individuals with developmental or intellectual disabilities

• Strong clinical assessment and care coordination skills

• Excellent communication and documentation abilities

• Ability to travel between sites in Brooklyn and Queens

Nice-to-Have Qualifications

• Experience working in community-based residential programs

• Familiarity with OPWDD regulatory standards

• Experience training or supporting direct support staff

• Case management or care coordination experience
