Trach LPN or RN: URGENT Private Duty Home Health

We are urgently looking for a full-time day nurse to provide 1:1 skilled care to a 11-year-old client that lives in Hudson and also needs the nurse to attend school when in session. We also have full-time and prn days/overnight shifts available, too! Cares would be provided at home when the client is not in school. This client does have high-acuity needs including g-tube feedings, trach cares and ventilator support. We will provide hands-on training. Must have trach experience.

Schedule needed:

Full-time and PRN
Overnights and Days available
Up to 10 hour shifts

Compensation:
LPNs: $27 - $32
RNs: $29 - $35
$2 shift differential for hours between 7pm and 7am
Paid weekly

Our Registered Nurses and Licensed Practical Nurses work in our clients' home providing skilled medical care. Schedules are based on the client and the family needs. No two clients are alike. Key responsibilities include but are not limited to:

• Complete assessments, medication administration, and provide therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician’s orders.
• Partner with all members of the interdisciplinary team including the family, ensuring a constant flow of communication regarding the client’s cares, changes in condition, developmental goals, etc.
• Provide support and education to clients and families to help them be successful.
• Build rapport and trust with clients, families or other caregivers, the office team, and peers to facilitate effective communication and build relationships.
• Complete timely charting through our electronic medical record system.
• Keep up to date with the client’s plan of care, changes in conditions, physician’s orders, family, or caregiver requests, etc.
• Advocate for the client and family to promote and support growth and development.
• Examples of some of our nursing cares include gastrostomy tubes, jejunostomy tubes, nasogastric tubes, PICCs, PORTs, tracheostomies, seizure disorders, etc.

What’s required?

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure, without restrictions
• CPR certification required but may be obtained after hire
• Ability to lift, carry, push, or pull up to 50 pounds
