POSITION PURPOSE:

The primary purpose of this position is to provide MnCHOICE assessments as part of the eligibility determination process for adults and children to access Home and Community Based Services (CADI/CAC/BI/DD Waivers, Rule 185 Case Management, PCA Services) and/or to determine needs and make referrals for other support services as determined by this assessment process. Duties include, but are not limited to, determining eligibility, assessment of strengths and needs, helping individuals in person-centered planning, arranging, coordinating and monitoring services all in accordance with applicable Rule, Statute, any other federal or state regulations and Agency policy.

This is a part time, non-benefited position where the work demand can fluctuate based on the New Assessments and Reassessments case load each month.

 

EXAMPLES OF DUTIES:

  • Conducts comprehensive assessments for individuals by evaluating their physical, mental, social and environmental health status.
  • Determines eligibility for home and community-based services (CADI/CAC/BI/DD Waivers, Rule 185 Case Management and PCA Services).
  • Determines individual's level of care needs according to state and federal criteria.
  • Knowledge of appropriate diagnostic and financial eligibility.
  • Conducts reassessments to determine ongoing eligibility and service need changes for clients currently receiving home and community-based services.
  • Assessment of the client's strengths and needs to determine appropriate service options that can be delivered in the least restrictive environment.
  • Collaboration with case managers and service providers to gain a thorough understanding of the individual's service needs.
  • Development of person-centered support plans that identifies individualized goals.
  • Maintains a broad knowledge base of payer sources and community-based services.
  • Interprets program options for individuals and provides referrals to appropriate community resources.
  • Maintains the appropriate certification as required by state regulations.
  • Travels to home and community settings to complete assessments.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of the principles and techniques of interviewing and recording in social work and the ability to apply them.
  • Knowledge of individual and group behavior.
  • Knowledge of social casework objectives, principle and methods.
  • Knowledge of social-economic factors which promote stable independent community living.
  • Knowledge of the principles of community organization.
  • Knowledge of physical and mental illnesses.
  • Knowledge of culture of poverty, social systems, and impact of trauma.
  • Knowledge of computer and word processing skills.
  • Ability to effectively apply casework knowledge and skills.
  • Ability to work constructively within an agency in the community setting and in effectively utilizing appropriate resources and services.
  • Ability to work constructively in the development and coordination of community resources to meet special needs.
  • Ability to manage time productively.
  • Ability to prioritize caseloads.
  • Ability to communicate effectively, both orally and in writing.
  • Must possess valid driver's license.
  • Must pass background investigation.
