Complete Care is seeking an experienced and relationship-driven Registered Nurse (RN) to join our team as a PRN Nurse Liaison supporting our Connecticut region. This role focuses on external case management, hospital partnerships, and referral development, stepping in to provide coverage during vacancies, vacations, and leaves of absence.

This is an ideal opportunity for an RN who enjoys working outside of a traditional facility setting, building relationships, and facilitating smooth patient transitions into post-acute care.

Key Responsibilities

  • Serve as a clinical liaison between hospitals, case managers, and Complete Care facilities

  • Build and maintain strong relationships with hospital discharge planners, case managers, and referral sources

  • Review clinical referrals and assess appropriateness for admission to skilled nursing facilities

  • Coordinate and facilitate patient transitions from acute care settings to Complete Care centers

  • Provide clinical insight and support to ensure appropriate level of care and successful placements

  • Maintain consistent communication with internal admissions and clinical teams

  • Represent Complete Care in the field, promoting services and strengthening referral pipelines

  • Provide coverage across the region as needed to support business continuity

Qualifications

  • Active Registered Nurse (RN) license (Connecticut required; multi-state licensure a plus)

  • Prior experience in case management, nurse liaison, admissions, or discharge planning strongly preferred

  • Background in long-term care, skilled nursing, or acute care required

  • Strong understanding of post-acute care criteria and insurance/Medicare guidelines

  • Excellent interpersonal and relationship-building skills

  • Highly organized with the ability to manage multiple referrals and priorities

  • Willingness to travel throughout the Connecticut region
