Description
Position Overview
Under minimal supervision, the Case Coordinator will work independently and together with the CalAIM Team and community agencies to provide coordination support to the CalAIM department, building a foundation for outreach and direct case management services to help clients achieve housing outcomes and wellness and recovery goals. The Case Coordinator plays a crucial role by supporting the Community Support Navigator and or the Case Manager- ECM with scheduling, documentation, tracking and data collection. This position requires consistent and proactive communication with CalAIM Team members to ensure smooth service delivery for its members. Additional duties may be assigned as needed and could include supporting other departments under the direction of other supervisors or managers.
Expected Hours and Place of Work: This is not a remote position. The incumbent works full-time, primarily at their assigned workplace, where the typical office hours are Monday-Friday 8:00 am-4:30 pm, and some evenings and weekends as necessary for optimal business function.
Key Responsibilities:
Scheduling & Care Coordination
- Develop and maintain positive professional relationships with various stakeholders to help clients achieve their goals and maintain City Net’s positive reputation in the community.
- Coordinate care and information across providers and support members.
- Collaboration with partner agencies and/or community resources for best outcomes
- Track members attendance to appointments
- Check member eligibility monthly or as needed
Documentation
- Monitor and assist with activities entered in SPRITE and identify any service gaps
- Maintain client files in compliance with all MCP and DHCS requirements
- Monitor HMIS service entries for accuracy and current enrollment
- Submit Housing Deposit requests according to each MCP’s guidelines
- Assist in documentation for Debit Card Logs and Finance folders for assigned members
Tracking Referrals & Authorizations
- Check MCP Portals, fax and email daily for authorizations and referral activities.
- Maintain communication with CalAIM team regarding updates on authorizations
- Notify team and assign member once member is authorized
- Maintain MCP trackers in coordination with supervisor and billing team.
File Quality & Maintenance Tracking & Collection
- Audit Caseload list and Sprite caseload for accuracy monthly
- Attend Case Conferencing to remain up to date on cases
Data Collection
- Run regular reports on demographics or outcomes as needed for any subgroups
- Ensure appropriate data is being collected in accordance with MCP and City Net goals.
Requirements
Qualifications:
- Associates degree or higher with at least one year experience in healthcare and/or casework, or equivalent combination of education and experience
- Experience cultivating and maintaining productive, professional relationships with various stakeholders
- 1 to 2 years of customer service experience, preferable in social services or public health
- The ability to problem solve independently as well as the ability to collaborate in a team setting to work toward a specific goal
- Understanding of the importance of quality and timely data collection and reporting.
- Excellent verbal and written communication skills.
- Excellent professional boundaries.
- Strong interpersonal, analytical, and time management skills.
- Organized with confidential client material and appointment tracking
- Flexible and adaptable in response to changing client and health care providers’ needs
- Experience or interest in working with underserved and marginalized populations
