Job Title: Speech Language Pathologist SLP

Job Summary:

Join Pearl Pointe, a leading provider of Skilled Nursing services in Freeport, Illinois, as a Speech Language Pathologist SLP! We are committed to providing high-quality care and support to our residents, and we are seeking a dedicated and compassionate SLP to join our team.

Responsibilities:

  • Provide speech therapy services to residents with various communication disorders and cognitive impairments

  • Develop and implement individualized treatment plans to achieve optimal communication and cognitive function

  • Conduct assessments, screenings, and evaluations to identify resident needs and develop strategies for improvement

  • Collaborate with other healthcare professionals to ensure comprehensive care and support

  • Maintain accurate and detailed records of resident progress and treatment plans

  • Participate in quarterly care plan updates and quarterly progress reports

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program

  • Illinois Speech-Language Pathology licensure

  • Minimum 0 years of experience preferred

  • Strong communication and interpersonal skills

  • Ability to work collaboratively as part of a interdisciplinary team

  • Familiarity with Medicare and Medicaid regulations and guidelines
