Job Title: Speech Language Pathologist SLP
Job Summary:
Join Pearl Pointe, a leading provider of Skilled Nursing services in Freeport, Illinois, as a Speech Language Pathologist SLP! We are committed to providing high-quality care and support to our residents, and we are seeking a dedicated and compassionate SLP to join our team.
Responsibilities:
Provide speech therapy services to residents with various communication disorders and cognitive impairments
Develop and implement individualized treatment plans to achieve optimal communication and cognitive function
Conduct assessments, screenings, and evaluations to identify resident needs and develop strategies for improvement
Collaborate with other healthcare professionals to ensure comprehensive care and support
Maintain accurate and detailed records of resident progress and treatment plans
Participate in quarterly care plan updates and quarterly progress reports
Requirements:
Master's degree in Speech-Language Pathology from an accredited program
Illinois Speech-Language Pathology licensure
Minimum 0 years of experience preferred
Strong communication and interpersonal skills
Ability to work collaboratively as part of a interdisciplinary team
Familiarity with Medicare and Medicaid regulations and guidelines
