Overview:

The Community Health Social Care Network (SCN) Coordinator is a detail-oriented, relationship-driven professional with strong administrative and organizational skills. The Community Health SCN Coordinator is responsible for overseeing the overall operation of the SCN program, conducting standardized social needs screenings, engaging members to identify unmet social determinants of health (SDOH), and facilitating referrals and linkages to appropriate community-based services within the SCN. This role supports members in navigating services related to food insecurity, housing instability, transportation, utilities, and other social needs, while ensuring accurate documentation and compliance with NY Medicaid and SCN requirements. You will collaborate with other local community-based organizations, support SCN Screening staff in their duties, and engage with members to share information about the program.  Your networking and community outreach skills will make a direct, positive impact on our community.

Responsibilities:
  • Conduct standardized SCN social needs screenings (e.g., food, housing, utilities, transportation, interpersonal safety, etc.)
  • Engage members telephonically and/or in person using trauma-informed and culturally competent approaches
  • Identify eligible SCN services and determine appropriate next steps based on screening results
  • Explain available social care services and member rights in clear, understandable language
  • Collaborate with Marketing Manager to maintain and expand community outreach efforts
  • Initiate and coordinate strategic partnerships with local community-based organizations
  • Support screeners in completing screenings, maintaining compliance with NY Medicaid and SCN requirements and overcoming barriers to delivery of program service
  • Track client referral statuses and follow up as needed to ensure referrals have been accepted
  • Track client invoice statuses to ensure all invoices have been generated, submitted, and paid
  • Attend all required SCN meetings and coordinate with supervisory staff to ensure all required administrative deadlines are met
Qualifications:
  • Bachelor’s degree in a related field required; graduate-level coursework or advanced degree preferred. Equivalent experience considered.
  • One to three years of administrative experience and experience working with staff
  • Knowledge of Microsoft Excel, Word, Publisher, and PowerPoint
  • Excellent interpersonal communication skills - written and oral
  • Excellent project management and organizational skills
  • Must have reliable transportation

  • Within 30 days of hire, completion of Child Abuse Prevention, NYS Sexual Harassment Training, Introduction to the Y’s Cause and Culture, and all SCN Unite US platform traning requirements
  • Ability to relate effectively to diverse groups of people from all social and economic segments of the community
  • Commitment to inclusion and compliance with the Americans with Disabilities Act (ADA)
  • Bilingual preferred
