Job Summary:

We are seeking a compassionate and skilled LPN to join our day shift team at St Johnsbury Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider in Saint Johnsbury, Vermont. As a valued member of our team, you will play a critical role in delivering exceptional care to our patients and residents.

Responsibilities:

• Provide direct patient care under the supervision of our nursing staff, including administering medications, wound care, and other treatments as needed

• Develop and implement individualized care plans to meet the unique needs of each patient

• Collaborate with our interdisciplinary team to ensure comprehensive care and promote positive outcomes

• Maintain accurate and detailed records of patient care and progress

• Participate in quality improvement initiatives and contribute to a culture of continuous learning and improvement

Requirements:

• Currently licensed as an LPN in the state of Vermont

• 0 years of experience in a healthcare setting (new graduates welcome to apply!)
