$25–$32/hour (DOE) + Mileage

SummitWest Care is seeking Licensed Practical Nurses (LPNs) interested in any level of availability (part‑time, PRN, or flexible weekday coverage). Any consistent time you can offer is valuable and helps support patient care and RN capacity.

This role is ideal for LPNs balancing multiple commitments or looking for meaningful, flexible work in their community.

Who We're Looking For:

• Self‑directed clinicians comfortable managing their own schedules
• Strong communicators who raise concerns early
• Calm, grounded nurses who adapt well to the realities of home health
• Team players willing to step in when coverage is needed

What You'll Do:

• Provide skilled nursing care in patients' homes
• Assist with wound care, medications, and patient education
• Communicate changes or concerns clearly
• Document visits accurately and on time in our EMR
• Collaborate with RNs, therapists, and caregivers

Qualifications

• Active Colorado LPN license
• IV certification strongly preferred, not required
• Strong communication and documentation skills
• Reliable transportation
