About the Role
We are seeking a highly organized, detail-oriented, and proactive professional to join our team as an Early Intervention Coordinator / Case Management Specialist. This role is ideal for someone who thrives in a fast-paced environment, enjoys problem-solving, and is passionate about supporting children and families through the Early Intervention (EI) process.
You will play a key role in managing case workflows, ensuring compliance, and supporting service coordination teams to deliver high-quality care.
What You’ll Do
- Create and submit referrals through EI Hub
- Open and manage cases in ProviderSoft, including document downloads and updates
- Maintain and update authorizations, plans, and case statuses
- Monitor transition logs and follow up on case progress with Service Coordinators (SCs)
- Manage case queues, assign cases, and track authorizations
- Schedule IFSP meetings with Brooklyn Regional
- Generate certificates and distribute to coordinators and providers
- Send certified letters to families as needed
- Review session notes for accuracy and compliance
- Run weekly and monthly reports (case status, authorizations, progress reports, etc.)
- Assist with audits and ensure regulatory compliance
- Provide ongoing support to the care team and collaborate across departments
What We’re Looking For
- High School Diploma required; Bachelor’s Degree preferred
- Early Intervention experience strongly preferred
- Strong attention to detail and organizational skills
- Ability to manage multiple tasks and deadlines in a fast-paced environment
- Proactive mindset with strong problem-solving abilities
- Collaborative team player with excellent communication skills
- Ability to remain calm and focused under pressure
