What You’ll Do:

Provide 1:1 therapy sessions in patients’ homes.
Evaluate and treat a primarily geriatric population.
Create and implement individualized care plans.
Maintain clear and timely documentation

Qualifications:

Licensed PT in the state of NJ.
Degree in Physical Therapy.
Strong communication and clinical skills.
Home health experience (preferred, not required).
