SummitWest Care is hiring a Full-Time Registered Nurse (RN) for our home health team. This role offers a consistent weekday schedule, manageable visit expectations, and the opportunity to build meaningful relationships with your patients.

What You'll Do

• Provide skilled nursing care in a home health setting

• Complete comprehensive patient assessments and develop individualized plans of care

• Monitor patient progress and adjust care plans as needed

• Educate patients and families on medications, treatments, and self-care

• Coordinate care with an interdisciplinary team (LPNs, therapists, caregivers)

• Complete documentation in Homecare Homebase (HCHB) – training provided

• Participate in a rotating on-call schedule

• Travel locally within Grand Junction, Fruita, Clifton, and Palisade

What You Bring

• Active Colorado Registered Nurse (RN) license

• Strong clinical assessment and critical thinking skills

• Ability to work independently while managing a structured caseload

• Clear, professional communication style

• Comfort with documentation and basic technology

• Reliable vehicle, valid driver's license, and auto insurance

• Home health experience preferred (we will train the right candidate)
