SummitWest Care is hiring a Full-Time Occupational Therapist (OT) to provide one-on-one care in patients' homes throughout Mesa County.

As a local nonprofit home health agency, we prioritize sustainable caseloads, ethical care, and clinician support-so you can do your best work without burnout.

What You'll Do

• Deliver skilled occupational therapy services in a home health setting

• Complete evaluations, reassessments, recertifications, and discharge visits

• Create personalized treatment plans focused on safety, independence, and daily function

• Perform home safety evaluations and recommend adaptive equipment and modifications

• Educate patients and caregivers on ADLs, mobility strategies, and home programs

• Collaborate with an interdisciplinary care team (RNs, PTs, caregivers, and families)

• Supervise and support a COTA, when applicable

• Complete documentation in Homecare Homebase (HCHB) – training provided

• Travel locally within Grand Junction, Fruita, Clifton, and Palisade

What You Bring

• Active Colorado Occupational Therapist (OT) license

• Ability to work independently in a home health environment

• Strong clinical decision-making and time management skills

• Professional communication and patient-centered approach

• Reliable vehicle, valid driver's license, and auto insurance

• Home health experience preferred (we will train the right candidate)

• A genuine passion for helping patients maintain independence at home
