SummitWest Care is seeking a Part-Time or PRN Speech-Language Pathologist (SLP) to provide one-on-one therapy in the home health setting across Mesa County.

What You'll Do
-
Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders
-
Develop and implement individualized, functional treatment plans
-
Provide dysphagia management and cognitive-communication therapy
-
Educate patients and caregivers on strategies to improve communication, safety, and independence
-
Collaborate with an interdisciplinary team (RNs, PTs, OTs, social workers, caregivers)
-
Complete documentation in Homecare Homebase (HCHB) – training provided
-
Travel locally within Grand Junction, Fruita, Clifton, and Palisade
What You Bring
-
Active Colorado Speech-Language Pathologist (SLP) license
-
CCC-SLP preferred (CFs may be considered depending on experience 👀 optional if you want to open pipeline)
-
Strong clinical judgment in cognitive and swallowing disorders
-
Ability to work independently in a home-based setting
-
Clear, compassionate communication style
-
Reliable vehicle, valid driver's license, and auto insurance
-
Home health or medical experience preferred (we will train the right candidate)
