Essential Functions:

Client Care & Support

  • Assist nurses with health screenings, assessments, and vitals

  • Support coordination of physical and behavioral health services

  • Provide client education on wellness, medications, and symptom management

  • Support outreach, engagement, and follow-up services

  • Respond to on-call needs as required

Prescriber & Medication Support

  • Coordinate labs, refills, and prior authorizations

  • Assist with medication administration, including injections (as appropriate)

  • Support medication room organization, compliance, and inventory

  • Communicate assessment findings to nursing staff and prescribers

Team Collaboration & Documentation

  • Participate in a multidisciplinary treatment team

  • Maintain timely, accurate, person-centered documentation

  • Support quality improvement and ongoing professional development
