$25–$32/hour (DOE) + Mileage
SummitWest Care is seeking a Licensed Practical Nurse (LPN) to support our home health program full-time. This role offers a consistent weekday schedule and the opportunity to provide one‑on‑one care in the homes of local clients.
This position is a good fit for an LPN who is self‑directed, communicates clearly, and is comfortable managing much of their own day in the field.
Who We're Looking For:
- Organized, dependable clinicians who work well independently
- Calm, steady professionals who adapt well to unpredictable situations
- Strong communicators who share concerns early and clearly
- Team‑oriented nurses willing to support colleagues when needed
What You'll Do:
- Provide skilled nursing care in patients' homes
- Perform wound care, medication management, and disease education
- Complete orthopedic exercises assigned by Physical Therapy
- Administer IV antibiotics and draw labs (within scope/training)
- Communicate patient changes to RNs and leadership
- Complete timely, thorough documentation in our EMR (training provided)
Home health happens in real homes, not controlled clinical settings.
Qualifications
- Active Colorado LPN license
- IV certification preferred, not required
- Strong communication and documentation skills
- Reliable transportation
