Essential Functions & Responsibilities:
The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well being, and evaluates the patient's progress.
The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well being, and evaluates the patient's progress.
Provides appropriate and thorough teaching to patients and their caregivers.
Participates in staff and administrative meetings.
Qualifications:
- Must be currently licensed by the Florida Board of Physical Therapy to practice physical therapy in the state of Florida.
- Must have a FL driver's license, vehicle insurance, and access to a dependable vehicle.
- Must pass AHCA Clearinghouse background check (https://info.flclearinghouse.com)
- Current in person CPR certification required
- Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting patients and /or equipment.
- Customer service abilities including effective listening skills.
Experience:
- Physical Therapy: 1 year (Preferred)
- Home Health: 1 year (Preferred)
Pay: Commensurate with PT experience, home health experience, and documentation knowledge.
