Cornerstone Care has locations throughout Southwestern Pa. and we are seeking a Referral Specialist to join our team for our Mount Morris location.

Communicates effectively with a broad range of patients, community and organizational members.
Responsibilities Include:
- Schedules appointments and handles referrals for patients per providers request to outside entities.
- Handles referral in-house testing with contracted facilities and specialists.
- Completes referrals for various insurances.
- Handles emergency and urgent referrals, when requested.
- Receives all test reports and letters from specialists and notifies the providers. Alerts the provider of any urgent reports.
- Sends all pertinent information for appointments.
- Contacts patients with information regarding referrals and any specific instructions
- Communicates with all provider relations representatives and contacts them with any concerns, questions, problems or needs.
- Documents referrals on patients medical records.
- Documents needed testing or follow up on procedures and testing.
- Tracks diagnostic tests and reports.
- Communicates effectively with a broad range of patients, community and organizational members. Must be highly organized and able to multi-task as necessary. Must be able to perform efficient and accurate processing of patient visits including insurance verification, scheduling and data entry while maintaining positive patient/visitor relations.
