Medical Adult Daycare Driver

TRI-STATE LIFECARE MEDICAL ADULT DAY CARE DRIVER

Job Title: Medical Adult Daycare Driver
Reports to: VP Clinical Services
FLSA Status: Non-exempt

Essential Functions:

• Responsible for maintaining a schedule of transportation to and from the center which includes keeping meticulous records of the pick-up and drop-off of participants.
• Assists the participant out of the home and into and out of the vehicle, as necessary.
• Reports to program director immediately if a participant does not appear for previously scheduled transportation.
• Makes effort to determine if the participant is helpless and unable to answer the door or the phone or has gone away for the day.
• Maintains a schedule of maintenance of the vehicle, including documented safety checks of the safety devices.
• Must be physically able of performing the required duties and of assisting frail, elderly, and disabled adults, as necessary.
• Able to read and write English, fluently
• Must provide the center with a health examination , signed by a physician and a TB clearance which is not more than 12 months old prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aide training, or shall complete training within six months of employment.
• Must pass a criminal background check as required and may not have criminal convictions or criminal history that indicates behavior that is potentially harmful to participants as evidenced through a criminal history records check.

Education and Work Experience:

• High School education or GED
  
• Must be at least 18 years of age
  
• Must have experience in transporting patients
  
• Training in emergency procedures
  

Certification and Licenses:

• Current CPR and first aid certification
• Must possess a valid driver's license
  • Employees and volunteers must show proof of current automobile insurance coverage and pass the Motor Vehicle Report, on an annual basis. To be considered acceptable, the following criteria must be met:
    • No more than three moving violations or more than one chargeable accident during the last three years. A chargeable accident is defined as any accident that results in the driver being cited with a violation or that the driver negligently contributes to the incident, or any single vehicle accident not caused by equipment failure
    • No major convictions within the past five years. Examples of this are as follows:
      1. Driving under the influence of alcohol or drugs
      2. Reckless driving
    • No license suspensions or revocations for major violations within the past five years
    • Drivers transporting participants must provide proof that they maintain minimum amount of personal automobile liability insurance required by their state of residency
    • An annual motor vehicle report must be checked indicating the employee has an "acceptable" driving record.

Work Environment and/or Physical Demands:
The MADC Driver sits, stands, bends, pushes and pulls, lifts, and drives intermittently throughout the day and while setting up and tearing down fund raising events. The MADC Driver must have sufficient strength to perform these duties.
