Certified Occupational Therapy Assistant (COTA) – Skilled Nursing Facility (SNF)
\nWe are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join a skilled nursing facility team providing short-term rehabilitation and long-term care services. This role offers the opportunity to work with a primarily geriatric population focused on restoring functional independence, improving safety, and enhancing quality of life.
\nThe COTA will implement individualized treatment plans developed by the supervising Occupational Therapist and play a key role in helping residents regain strength, mobility, and confidence following illness, surgery, or hospitalization.
\nKey Responsibilities:

Provide skilled occupational therapy interventions under OT supervision
\n

Assist residents with ADL retraining including dressing, bathing, grooming, toileting, and feeding
\n
- \n

Address functional mobility, balance, endurance, and safety awareness
\n

Implement fall prevention strategies and adaptive techniques
\n
- \n
- \n
Train patients in use of adaptive equipment and compensatory strategies
\n
- \n
- \n
Accurately document daily treatment sessions and progress in EMR
\n
- \n
- \n
Collaborate with interdisciplinary team including nursing, PT, SLP, and case management
\n
- \n
- \n
Maintain productivity standards while ensuring high-quality patient-centered care
\n
Ideal Candidate:

Active COTA license in practicing state
\n
- \n

SNF or geriatric experience preferred
\n
- \n
- \n
Strong time management and clinical reasoning skills
\n

- \n
Comfortable managing a varied caseload of short-term rehab and long-term residents
\n
