Position Summary:
Uphold the mission and beliefs of the organization by assuring that all patients receive high quality, comprehensive health education.This position includes, but is not limitedto:designing, promoting,implementingand evaluating comprehensive, high quality health education activities that focus on at-risk populations,in order toincrease positive health outcomes.This position is also responsible for evaluating the medical and social needs of these patients and connecting them with services as needed.
Qualifications
- Masters Degree preferred
- At least two (2) years as a registered/licensed Dietician
Knowledge, Skills, and Abilities
- Experienceproviding education to people among various ethnic and social backgrounds, experience in an outpatient practice setting preferred
- Excellentorganizational skills to managenumerouspractice activities
- Excellent interpersonal skills,team player, professionalism, responsible,courteousand reliable
- Proficiencyin computer skills,familiarity with Electronic Medical Records and willingness to learn all computer systems.
- Demonstratesinitiative,leadershipanda high levelof educator skills
- Unconditional ability tomaintainHIPAA regulations
- Demonstrates culturalcompetency among persons from various ethnic and social backgrounds
- Leadership skills thatinclude the ability and desire to champion teambuilding withinprimary Care.

Scope of Responsibilities:
- Work collaboratively with the primary care providers to identifyat-risk patients in need of health education as well as broader population needs around health promotion and disease prevention, includingdiabetes education, weight management, cardiovascular disease,hypertension,renal disease, malnutrition / weight gain, infants with failure to thrive, cancer and nutrition therapy, gastrointestinal disorders,nutrition therapy, sports nutrition or healthy diet, and other chronic diseases.
- Designand implement comprehensive, high quality group education programs for at-risk patients (and family members) to increase positive health outcomes and reducehigh riskbehaviors.
- Ensure content of group sessions are consistently evaluated to ensure high quality and standards of care
- Ensure measurable outcomes / data are collected and analyzed
- Provide servicesto patients of all ages,including:patient education, counseling,assistancein obtaining needed services and supplies.
- Providecomprehensive,individualized health education and self-management support to patients, in conjunction with theprimarycareproviders
- Advance the development,implementationand evaluation of the health education programs
- Participate in health education andpromotion activities in collaboration with the Community Health Educator, Director of Clinical Support, the Director of Patient and Community Services, and the Chief Medical Officer.
- Maintain andsubmitmonthlyreports to the Chief MedicalOfficerthat detail encounters, group sessions, and diabetic education activities
- Act as aliaison/representative at meetings, workshops, conferences as needed
- Developappropriate healtheducation materials and programs for use in the health center, in conjunction with the Chief Medical Officer
- Attendance and participationin meetings, in-services, educationalprogramsand other activities asdeemedappropriate
- Maintain current Health Center policies and guidelines
- Other duties and responsibilities as assigned
Physical demands:
- Travel isrequiredbetween site (s) by personal automobile or public transportation,
reimbursedin accordance withChesPenn Health Services reimbursement policy.
- Frequentand prolonged sitting,standingand/or walking may berequired.
- Moderate physical effortwithcarrying forms,suppliesand equipment.
- Climbing stairs may berequired.
- Stooping and stretching may berequired.
- High levelsof stress may need to be endured due to the diverse nature of the care provided.
Work environment:
- The primary workplace is a busy multi-cultural ambulatory care setting.There is routine exposure toinfectionsdisease and blood borne pathogens.
