Description
We are looking for a dedicated and compassionate Skilled Nurse to deliver high-quality, client-centered care. You will assess health needs, provide skilled nursing procedures, administer treatments, and coordinate with the healthcare team to promote optimal outcomes.

Responsibilities


  • Conducts comprehensive nursing assessments and develops individualized care plans based on client needs and physician directives.
  • Administers prescribed medications, treatments, and therapies in accordance with state laws and agency policies.
  • Monitors and evaluates clients physical, mental, and emotional conditions, reporting changes promptly to the physician and/or supervising nurse.
  • Provides wound care, catheter care, IV therapy, and other skilled nursing procedures as required.
  • Educates clients and family members on disease management, medication administration, and home care best practices.
  • Maintains accurate, detailed, and timely documentation of nursing visits and interventions.
  • Adheres to infection control, safety, and emergency protocols at all times.
  • Supports and supervises other care team members as appropriate within scope of practice.

Requirements

  • Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license issued by the Maryland Board of Nursing.
  • Minimum of one (1) year of nursing experience, preferably in home health, hospice, or a related clinical setting.
  • Proven ability to perform skilled nursing procedures, including wound care, IV therapy, and medication administration.
  • Strong knowledge of infection control, home safety, and emergency procedures.
  • Current CPR and First Aid certification.
  • Must pass a criminal background check and be free from health conditions that may pose a risk to clients or co-workers.
  • Demonstrated ability to follow written and verbal instructions, maintain client confidentiality, and uphold professional ethics.
  • Compassionate, patient-focused, and able to work independently with minimal supervision.
  • Reliable transportation and willingness to travel to client homes or facilities as needed.
