Ultimate Therapy is seeking a licensed Physical Therapist to support rehabilitation services for adults in a long-term care and post-acute setting. The position centers on restoring function, preventing decline, and improving daily living skills.

What You'll Do:

• Conduct initial and ongoing patient assessments
• Design goal-oriented treatment plans based on clinical findings
• Provide hands-on therapy to address strength, balance, and mobility
• Track outcomes and update care plans accordingly
• Instruct patients on proper techniques and home programs
• Collaborate with physicians, therapists, and facility staff
• Complete timely clinical documentation

Requirements:

• Current PT license in good standing
• Ability to manage a varied caseload
• Team-oriented approach with strong interpersonal skills
• Prior experience in a rehab or SNF environment is a plus
