Shift Type: 4x10 Day shifts, 7:30 AM - 6:00 PM, four 10-hour shifts per week with every-other-weekend coverage required (Day shift)

Requirements
- Coordinate safe, timely discharges and next-level care transitions for inpatients
- Conduct thorough needs assessments and order DME when appropriate
- Develop and manage individualized plans of care focused on optimal outcomes
- Document patient care and case management activities accurately in Epic
- Collaborate closely with physicians, nurses, social services, and other stakeholders
- Work across hospital areas, including Colby and Pacific campuses, to meet patient needs
- Support regulatory compliance related to CMS guidelines and utilization review
- Cover every-other-weekend shifts and adapt to floating between units as needed
- Active RN license required (pending licenses accepted) — MSW may be considered for certain case management roles
- BLS certification required
- Minimum 1 year acute hospital experience with discharge planning or care coordination
- Experience with case management tasks: needs assessment, DME ordering, plan-of-care development
- Comfort working with high patient ratios (experience with 1:25–30 days is noted)
- Willingness to float across units and work both Colby and Pacific campuses (campuses are <2 miles apart)
- Ability to work every other weekend as part of the rotation
