Job Description:
Home Health Services of Virginia is seeking versatile and dependable Field Nurses to provide high-quality care to patients in the home health setting. Field Nurses are a critical part of our agency and must have prior experience working in home health.

Qualifications:

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license from the Virginia State Board of Nursing.

• Experience working in a home care agency.

• Strong clinical, communication, and documentation skills.

Job Duties:

• Complete patient care duties according to the patient’s plan of care.

• Reevaluate and adjust care as necessary.

• Work scheduled shifts to perform patient duties.

• Complete clinical nurse notes in their entirety.

• Provide skilled care services as applicable.

• Perform additional duties as assigned.
