Compensation: $56,000 – $73,000 Base (approx. $27.00 – $35.00/hour) | Up to $100,000+ with Overtime Schedule: Monday–Friday + Required Rotating Weekends
Be the Heart of Our Patient Care OperationsFor nearly 20 years, Health Link Home Health & Hospice has been a leader in Bay Area healthcare. We are looking for a high-energy, detail-oriented Intake & Scheduling Coordinator to join our San Francisco team.
As the "Air Traffic Controller" of our office, you ensure that patients transitioning from hospitals to their homes receive care without delay. This is a fast-paced role that requires someone who can juggle logistics while maintaining a compassionate touch.
The Schedule: Consistency + FlexibilityTo provide world-class care, our office remains responsive seven days a week.
This is a dedicated, full-time position.
Your core hours will be a regular Monday through Friday shift.
Participation in a rotating weekend coverage schedule is essential to this role to ensure our patients always have support.

Process incoming patient referrals from hospitals and providers with speed and accuracy.
Work directly with patients and clinical teams to assign and schedule home visits.
Proactively troubleshoot scheduling conflicts to ensure continuity of care.
Maintain flawless records in our software systems and healthcare CRMs.
Minimum 1 year of experience in healthcare intake or scheduling (Home Health or Hospice experience is a major plus!).
Ability to speak professionally and empathetically with doctors, families, and clinicians.
You thrive in a "moving parts" environment and can pivot priorities quickly.
Comfortable navigating healthcare CRMs, digital scheduling platforms, and medical terminology.
