As an ACTT Case Coordinator, you’ll play a critical role in connecting youth and families to the services and support they need—guiding them through every stage of care from admission to successful discharge. This is a fast-paced, relationship-driven role where collaboration, advocacy, and compassion are key.

What You’ll Do
In this role, you’ll serve as a central point of coordination for youth in ACTT services:
Coordinate Care & Support Transitions
- Ensure smooth transitions into and out of ACTT services
- Collaborate with providers, families, and community partners to coordinate care
- Develop and support effective discharge plans that set youth up for long-term success
Advocate for Youth and Families
- Connect families to community resources, services, and supports
- Identify kinship placement options and partner with stakeholders (DSS, courts, families)
- Maintain consistent communication with all parties involved in a youth’s care
Facilitate Team Collaboration
- Lead or support Child and Family Team meetings
- Act as a liaison between internal teams and external partners
- Keep everyone aligned on goals, progress, and next steps
Support Compliance & Documentation
- Maintain accurate records and documentation within required timelines
- Assist with assessments, discharge documentation, and required reporting
- Ensure all authorizations, consents, and service records are complete and current

What We’re Looking For
- Licensed or provisionally licensed professional in a related field
- At least 1 year of experience working with children with serious emotional or behavioral needs
- Strong communication, organization, and problem-solving skills
- Ability to manage multiple responsibilities in a fast-paced, team-driven environment
- Valid North Carolina driver’s license and ability to travel for community-based work

Requirements:Education and Experience Requirements
- Licensed or Provisionally Licensed Professional Provider in the field of population served.
- Must have one year’s experience working with children with serious emotional disturbance.
- Requires effective communication, decision making, analytical, interpersonal, leadership and professionalism skills and abilities.
- Must have basic computer and math skills.
- Must have a valid North Carolina’s Driver’s License.
- Must submit to and pass pre-employment drug screening, criminal, HCPR and driving checks.
- Must submit personal insurance and registration information if utilizing personal vehicle for client transports.

- Employee must be able to operate a vehicle for job duties, and keep a valid NC drivers license with insurance.
- The work environment will contain slight to moderate office-related noises. The employee is not exposed to extreme weather conditions, toxic fumes, or airborne particles.
- The employee must occasionally travel to different locations in the course of work.
