The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

• Provide comprehensive case management services to patients.

• Document patient care and progress accurately using CERNER, NATE, and EnsoCare systems.

• Maintain open and effective communication with patients, families, and team members.
\n
• Adapt to changing priorities and multitask efficiently in a fast-paced environment.

• Build and maintain trust with patients and families to support optimal care outcomes.

\n\nQualifications\n

\n
• Current Registered Nurse (RN) license in the state of North Carolina or eligible for licensure.
\n
• Proven experience in case management or a related field.
\n
• Excellent documentation and clinical skills.
\n
• Strong communication and interpersonal skills.
\n
• Ability to work autonomously and within a collaborative team environment.
\n
• Experience with electronic health record systems preferred.
