- Valid and unrestricted Physical Therapist license in Vermont
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Minimum 1 year of experience in a Skilled Nursing Facility (SNF) setting
- Current BLS certification through American Heart Association (AHA)
- Clean professional license with no current or prior disciplinary action
- Strong interpersonal and communication skills with commitment to patient-centered care
