Physician Medical Director - Competitive Salary

Post acute care team in upstate New York, only half hour from Syracuse, is seeking a medical director. This is a full time opportunity rounding and providing leadership in SNFs with excellent support. Provide high quality care to a diverse patient population.  Collaborate with an experienced multidisciplinary team.  3 month onboarding program.  Perform comprehensive patient assessments, including medical histories and physical exams.  Develop, implement, and adjust individualized care plans based on patient needs.  Monitor progress and coordinate care to optimize outcomes.  Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.  Educate patients and families on health management and preventive strategies.  Maintain accurate, timely medical records in compliance with regulations.
