\nPrimary Care - Darby, part of Mercy Fitzgerald Hospital, is seeking a Board-Certified/Board-Eligible Family Medicine or Internal Medicine Physician to support continued primary care expansion and access initiatives across the Pennsylvania market.

- 100% outpatient no inpatient responsibilities \n
- Team-based care model (APP, support staff) \n

- Specialized care support including Endocrinology \n
The Opportunity
\n- \n
- Complex Chronic Disease Management: Diagnose and manage advanced, often co-occurring chronic conditions like poorly controlled diabetes, severe hypertension, substance use disorders, and asthma exacerbated by urban environmental factors. \n
- Social Determinants of Health (SDOH) Intervention: Screen for and address non-medical barriers to health, including food insecurity, housing instability, transportation deficits, and low health literacy. \n
- Trauma-Informed Care: Utilize trauma-informed communication strategies to care for patients experiencing chronic stress, chronic illness, or historical medical mistrust. \n
- Interdisciplinary Care Coordination: Collaborate closely with on-site social workers, case managers, community health workers, and behavioral health specialists to provide holistic patient support. \n
- Preventive Medicine & Harm Reduction: Administer critical immunizations, perform routine health screenings, and implement harm reduction strategies for high-risk behaviors. \n
- Resource Optimization: Navigate financial barriers by utilizing sliding fee scales, prescribing low-cost formulary medications, and leveraging local public health assistance programs. \n
