Primary Care Geriatrics Physician
Acclaim Multi-Specialty Group | Fort Worth, Texas

This position focuses on outpatient primary care with a geriatrics patient population.

Essential Duties and Responsibilities

  • Coordinate multidisciplinary care for assigned patient panels to optimize outcomes
  • Review patient medical records to diagnose, treat, and provide care management and education to patients and families
  • Make appropriate patient referrals as needed
  • Champion a culture of empathy, dignity, and respect for aging populations
  • Monitor national health guidelines and those established by JPS Health Network and Acclaim Multi-Specialty Group for patient care management
  • Provide evidence-based, compassionate care to improve patient outcomes and increase patient satisfaction scores while demonstrating patient-centered behaviors
  • Maintain a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, and support staff to ensure quality patient care
  • Support academic, educational, and research endeavors
  • Assess and manage chronic and acute conditions in patients with multiple comorbidities
  • Partner and communicate effectively to support a positive brand and patient/customer experience for JPS Health Network and Acclaim Multi-Specialty Group
  • Serve as a resource to the community
  • Perform other job-related duties as assigned

Qualifications

Required Education and Experience

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited school of medicine
  • Residency and/or fellowship training in Family Medicine (Geriatrics)

Required Licensure and Certification

  • Current, unrestricted medical license to practice in the State of Texas
  • Board Certified or Board Eligible
  • Unrestricted authorization to work in the United States

Preferred Qualifications

  • Bilingual or multilingual proficiency
  • Prior experience in post-acute or long-term care settings
  • Proficiency with electronic medical records and knowledge of CMS/SNF regulatory requirements

  • Ability to work comfortably in a large public/county healthcare facility
  • Demonstrated knowledge and competency in assessment techniques, diagnosis, and treatment processes within the assigned specialty area
  • Strong interpersonal, verbal, and communication skills to collaborate with healthcare professionals and serve as a community resource
  • Self-directed with the initiative to participate in community activities and medical outreach programs related to the specialty area
