- Coordinate patient care throughout hospitalization from admission through discharge. \n
- Develop, implement, revise, and evaluate individualized case management plans. \n
- Collaborate with physicians, nurses, social workers, therapists, and multidisciplinary healthcare teams to optimize patient outcomes. \n
- Assess patients' clinical, psychosocial, financial, and discharge planning needs. \n
- Utilize Epic and Cerner electronic medical record systems for documentation, care coordination, and patient tracking. \n

- Previous Case Management/Care Coordination experience preferred \n
