Physical Therapist — Home Health — Dublin, OH
Join a home health team in Dublin, Ohio as a traveling Physical Therapist. Provide patient evaluations, develop and implement individualized treatment plans, and deliver therapeutic services according to physician orders. Supervise and evaluate assigned Physical Therapy Assistants and Home Health Aides. Conduct home environment evaluations, document patient progress, participate in discharge planning, and collaborate with interdisciplinary team members to ensure continuity of care.
Minimum Qualifications
- Successful completion of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctoral degree) approved by the appropriate accrediting body.
- Current, unencumbered Physical Therapist license for the state of assignment prior to start.
- Current CPR certification.
- Experience in home health preferred. Must have reliable transportation and a valid driver’s license.
- Familiarity with electronic documentation and standard computer functions, including e-mail and basic navigation of software applications.
- Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
- State-specific requirements will apply and must be met for assignment (for example, supervision and documentation standards applicable to home health therapy services).
- 2–3 years of clinical physical therapy experience preferred.
Preferred Qualifications
- Previous home health travel experience.
- Experience conducting home safety evaluations and developing home-based treatment programs.
- Familiarity with state regulations governing home health therapy services.

Impacting Patient Care
Provide skilled physical therapy in patients’ homes in accordance with the physician’s plan of care. Perform initial and ongoing evaluations, develop and update individualized treatment plans, document outcomes, and coordinate care with physicians and interdisciplinary team members. Participate in in-service education and ensure compliance with applicable state regulations and agency policies. Contribute to improved patient function, safety, and discharge planning through evidence-based interventions and patient/family education.
