Home Health Aide

Horizons Community Care of Holiday Health Care is adding a Home Health Aide (HHA) to our team! We are looking for someone who has evening shift availability. A home health aide will help care for pediatric or adult patients with chronic illnesses or disabilities. Our ideal applicant is self-motivated, outgoing, and reliable.

Responsibilities:

• Follow the written instructions for patient care
• Perform simple procedures as an extension of therapy services
• Give personal care to patients including, but not limited to, bathing, shampoo, shaving, mouth care and skin care
• Assist patient with ambulation, transfers, and exercise
• Perform household services essential to health care at home
• Care for patient’s surroundings as necessary to maintain a safe environment
• Give reminders on medications that are ordinarily self-administered
• Report changes in the patient’s condition and needs to the Registered Nurses/Clinical Manager
• Recognize and report changes in skin condition including new wounds and pressure ulcers

Qualifications:

• Must have a valid CNA/HHA in the state of Indiana
• High school graduation or equivalent is required
• Must have a valid driver’s license and proof of auto insurance and ability to drive to patient residencies
• A minimum of one year of relevant experience in a nursing home, hospital, or caregiving is preferred
