RESPONSIBILITIES:

  • Provide intensive care coordination activities for clients
  • Ensure that clients have access to, engage in and retain needed services as defined in their care plan which may include such services as: Acute Medical Care, Primary Medical Care, Preventative Medical Care Services, Home Health Care, Behavioral Health Services, Community Social Support Services, Housing & State and Federal entitlements
  • Advocate for client services by phone and in person as needed
  • Perform home visits and follow-up to monitor services and client status
  • Participate in case conferences and trainings
  • Maintain relationships with service providers
  • Maintain accurate and timely documentation including all documents in client's charts, records and billing activity logs
  • Attend supervision with Program Director as per agency policy

 

 

QUALIFICATIONS:

  • Requires High School Diploma or GED with 2 years of care management experience
  • Experience & knowledge working with homeless, behavioral health (including mental health and/or addictions) and HIV populations required
  • Bilingual English/Spanish preferred, with good verbal skills in Spanish
  • Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues
  • Clean valid driver's license required
