Job Description:
We are seeking an experienced Home Health Hospice Case Manager to provide compassionate, patient-centered care in a home setting. The ideal candidate will have a strong background in home health, hospice, and case management, with the ability to manage patient care plans, coordinate services, and support patients and families during end-of-life care.

Key Responsibilities:

Manage and coordinate hospice patient care in a home health setting
Perform comprehensive patient assessments and develop individualized care plans
Utilize OASIS for accurate documentation and compliance
Document all patient care using Homecare Homebase (HCHB)
Provide after-hours support, including on-call responsibilities and urgent patient needs
Collaborate with interdisciplinary teams to ensure continuity of care
Educate patients and families on care plans, medications, and disease progression
Ensure compliance with all regulatory and agency standards

Requirements:

Active RN license
Required experience in Home Health, Hospice, and Case Management
Proficiency with OASIS documentation
Experience with Homecare Homebase (HCHB) required (must be listed on profile)
Strong clinical, communication, and organizational skills
Ability to work independently during after-hours coverage

Preferred Qualifications:

Prior travel experience
BLS Certification
