\n \nOpportunity:
\nRN - Home Health Nurse
\nJob Details:
\n• 13-week contract starting ASAP
\n • 36 guaranteed hours per week
\n • 8-10 hour day shifts
\n • Every 3rd weekend and holiday rotation required - weekend coverage subject to change based on nursing pool needs
\n • Every other holiday rotation required
\n • On-call every 4th weekend and 1 day during the week
\n • Average of 5 patient visits per day with potential for up to 6 visits depending on territory, patient census, and staff availability
\n • Travel up to a maximum of 150 miles in a given day (varies by day)
\n • Perform Oasis admissions, all time points, and episode management
\n • Responsibilities include physician calls, coordination of care, and timely documentation in EMR
\n • Clinical competencies required in wound care, IV, ports, lab draws, and chronic disease management
\n • Oasis charting experience required; Epic preferred
\n • Minimum 2+ years of home health experience preferred
\n • Required certifications: AHA BLS
\n • Active Michigan and Wisconsin nursing license required
\n • Valid driver's license required
\n • Associate Degree or Certificate required; Bachelor's Degree preferred
