Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

\n\nResponsibilities\n

\n
• Coordinate patient care, ensuring timely and effective care pathways.
\n
• Organize and participate in family meetings to discuss patient care plans.
\n
• Collaborate in the development and implementation of treatment plans.
\n
• Manage necessary medical equipment, SNF, and ambulance transport independently.
\n
• Float between units as needed based on staffing requirements.
\n
• Utilize Epic for comprehensive charting and patient data management.
\n

\n\nQualifications\n

\n
• Current RN license in the state of California.
\n
• Minimum of 2 years of case management experience in a clinical setting.
\n
• Demonstrated experience with Epic charting system is essential.
\n
• Experience in a variety of care settings including acute, long-term care, and rehabilitation facilities preferred.
\n
• Ability to work weekends as required and float between like units.
\n
• Current BLS certification is required.
\n
