- Position: RN PCU (Progressive Care Unit) at Trinity Health Livingston Bldg B
- Job Title: Livingston PCU 7p-7a shift

Job Details
- American Heart Association BLS certification required for all direct patient care personnel
- ACLS certification required for RNs in CCU, ED, PACU, MICU, SICU (included here as PCU is a progressive care unit)
- State nursing license verification must be dated within 30 days of start
- Trinity Specialty Specific Competency Testing must be completed within a year
- Trinity Code of Conduct Form must be signed
- Trinity I9 form completed by agency
- Confidentiality and Security Agreement signed
- Integrity and Audit Services Form signed prior to start
- Trinity Criminal Background Check within last 7 years, dated within 30 days of start
- Education verification of highest academic degree required
- EPLS and GSA/OIG exclusion reports dated within past 30 days
- Joint Commission CORE Exam within a year
- NSO report dated within 30 days of start, including statewide for MI
- System Access Form completed and uploaded one week before start
- Fit Test required
- Trinity 10 Panel Drug Screen within 30 days of start
- Proof of COVID-19 vaccine or approved declination/exemption
- MMR vaccination proof or IgG titer required
- Hepatitis B vaccination series must be started prior to start date
- Influenza vaccine proof or declination optional
- 2 Step PPD or IGRA test required with chest xray if positive
- Varicella vaccination proof or IgG titer required
- Physical/Health Statement within last year
- Tdap vaccine or declination accepted if no pediatric interaction
- Trinity Respirator Evaluation with physician signature required
- Trinity Skills Checklist provided by agency for clinical contractors
